Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 4/20/2023 with the provider and one assistant. There were 9 children present with 2 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of two children's records, the provider failed to have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child 1 and Child 2 each have diagnosed food allergies, but did not have an allergy action plan on file.
Plan of Correction: Provider will request allergy action plans for the children.
