Inspector Notes

A monitoring inspection was conducted on October 12, 2023 with the provider and assistants. There were 12 children present, ranging in ages from 3 years to 9 years, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 caregiver/household records were reviewed. The children were waking from rest, eating afternoon snack, and arriving from school.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on review of five children?s records, the provider failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 5 (date of enrollment 7/28/2021) did not have documentation of a physical until 1/19/2022.
Plan of Correction: Provider will get a physical from parents within 30 days of enrollment.
Standard 8VAC20-800-60-A
Based on review of five children's files, the provider failed to have required information in each child's file. Evidence: Child 1 and Child 2 did not have documentation of the name, address, and phone number of a second (non parent) emergency contact.
Plan of Correction: Provider will get a second non- parent emergency contact for each child.
