Inspector Notes

An unannounced renewal inspection was conducted today between 12:45pm and 2:00pm. There were 14 children (31 points) present with the Provider and 2 assistants. The children were observed being held and taking a nap. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, menu and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-800-180-A
Based on a review of household member records, the Provider did not obtain a subsequent report of tuberculosis screening every two years from the date of the first
screening or more frequently as recommended by a physician or the local health
department.

Evidence:
1. Household Member #2's most recent tuberculosis screening was dated 3/7/2020.
Plan of Correction: I will make an appointment for the screening.
Standard 8VAC20-800-40-A
Based on observation, the provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home.

Evidence:
1. There were 14 children receiving care in the family day home during the start of the inspection.
Plan of Correction: Two children are leaving and were here only for an emergency. I will only have 12 children.
Standard 8VAC20-800-780-B
Based on a review of children's records, 2 out of 4 children did not have acknowledgement of the review of the annual review of emergency contact information required to ensure the information is correct.

Evidence:
1. Child #1's (doe: 1/4/2018) last annual update of emergency contact information was on 7/7/2020.
2. Child #2's (doe: 1/4/2018) last annual update of emergency contact information was on 7/7/2020.
Plan of Correction: I will ask the parents to review and sign it today.
