Inspector Notes

An unannounced renewal inspection was conducted today between 12:15pm-1:10pm. There were 12 children present with the Provider and 2 assistant s for a total of 34 points within the supervision guidelines. The children were observed drinking bottles, finishing up lunch, toileting, and laying down for a nap. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Standard 8VAC20-800-100-A
Based on a review of children's records, the provider did not obtain documentation of a physical examination for 1 out of 4 children by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #4 (doe: 10/3/2022) did not have documentation of a physical examination on file at the time of the inspection.
Plan of Correction: I will ask the parents for it today.
Standard 8VAC20-800-90-A
Based on a review of children's records, the provider did not obtain documentation obtain documentation that all children have been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
1. Child #4 (doe: 10/3/2022) did not have documentation of immunizations on file at the time of the inspection.
Plan of Correction: I will ask the parents for it today.
