During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 2/6/2025. The on-site inspection began at 1:55pm and ended at 3:05pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 4 caregivers; the point total was 34. The inspector reviewed 5 children?s records and 5 caregiver/household members
records on-site on 2/6/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation
notice issued to the program.
Violations
4Caregiver #1 and Caregiver #2 did not have documentation of the results of a central registry background check on file at the time of the inspection.
Caregiver #1 had a TB screening on file that expired less than one month ago.
Household Member #1 and Household member #2 had a TB screening on file that expired less than one month ago.
A complete child record is required for each child in care.
Child #2 did not have a completed child record on file at the time of the inspection.
The family day home is required to obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Child #3's last documented immunizations on file were obtained 18 months ago.
Child #5's last documented immunizations on file were obtained 17 months ago.
