Inspector Notes
A monitoring inspection was conducted on 03/22/2022. At the time of entrance, there were 3 children present, ranging in ages from 19 months to 4 years with the provider.
The children were observed working with puzzles, tracing letters and engaged in free toy play.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There was no medication on site on this date. A total of 2 child records and 1 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
7Evidence:
1) The record for Child #2 (start date 12/09/2021) did not contain the results of a physical examination. 2) The provider could not locate the physical examination results for Child #2.
Evidence:
1) The record for Child #2 did not contain the addresses of two designated persons to contact in case of an emergency if the parent cannot be reached.
Evidence:
1) On the date of inspection, the first aid kit did not contain triangular bandages. 2) The provider stated that the triangular bandages were taken out of the first aid kit and have yet to be replaced.
Evidence:
1) The most recent review of the emergency information in the record for Child #1 was dated 10/19/2020.
Evidence:
1) On the date of inspection, the 2021 emergency drills log provided for review, did not have an evacuation (fire) drill documented for the month of August 2021.
Evidence:
1) The record for Child #2 (start date 12/09/2021) did not contain documentation of immunizations. 2) The provider could not locate the immunization record for Child #2.
Evidence:
1) The menu available for review was not dated for the current one-week period.
