Inspector Notes

A monitoring inspection was conducted on 03/22/2022. At the time of entrance, there were 3 children present, ranging in ages from 19 months to 4 years with the provider.
The children were observed working with puzzles, tracing letters and engaged in free toy play.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There was no medication on site on this date. A total of 2 child records and 1 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-100-A
Based on record review and interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The record for Child #2 (start date 12/09/2021) did not contain the results of a physical examination. 2) The provider could not locate the physical examination results for Child #2.
Plan of Correction: It is in his file now.
Standard 8VAC20-800-60-B
Based on records review, children's records did not contain all required information.
Evidence:
1) The record for Child #2 did not contain the addresses of two designated persons to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: Parents already did it.
Standard 8VAC20-800-760-A
Based on review of the emergency supplies and interview with the provider, the emergency kit did not contain all required components.
Evidence:
1) On the date of inspection, the first aid kit did not contain triangular bandages. 2) The provider stated that the triangular bandages were taken out of the first aid kit and have yet to be replaced.
Plan of Correction: I've already bought it.
Standard 8VAC20-800-780-B
Based on record review, the provider did not review with the parent, annually, the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The most recent review of the emergency information in the record for Child #1 was dated 10/19/2020.
Plan of Correction: Mother's already done it.
Standard 8VAC20-800-830-A
Based on documentation review, emergency evacuation procedures were not practiced monthly.
Evidence:
1) On the date of inspection, the 2021 emergency drills log provided for review, did not have an evacuation (fire) drill documented for the month of August 2021.
Plan of Correction: I am doing every month.
Standard 8VAC20-800-90-A
Based on record review and interview, the provider did not obtain documentation that the child has been adequately immunized before a child may attend the family day home.
Evidence:
1) The record for Child #2 (start date 12/09/2021) did not contain documentation of immunizations. 2) The provider could not locate the immunization record for Child #2.
Plan of Correction: They are in his file now.

Based on documentation review, the menu for the current one-week period was not dated.
Evidence:
1) The menu available for review was not dated for the current one-week period.
