Inspector Notes

A monitoring inspection was conducted on 03/22/2023 between the hours of approximately 9:00 am and 10:15 am. There were 3 children present, ranging in ages from 2 years and 3 years, with 2 caregivers present. Children were observed while playing with toy cars. Caregivers were supervising the children.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.035-B-2
Based on record review and interview, it was determined that the provider did not ensure that fingerprint-based criminal history search results are obtained every five years for caregivers.
Evidence:
1) The most recent fingerprint search request results available in the record for Caregiver #1 was dated 02/14/2018 and for Caregiver #2 was dated 02/15/2018.
Plan of Correction: The provider shall request the appointment to complete the fingerprints ASAP.
Standard 8VAC20-800-180-A
Based on record review, the provider did not ensure that caregivers obtain and submit the results of a follow-up tuberculosis (Tb) screening at least every two years.
Evidence:
1) The Tb screening form in the record for Caregiver #2 was dated 02/10/2021.
Plan of Correction: Caregiver #2 shall complete a Tb screening ASAP.
Standard 8VAC20-800-60-B
Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #2 did not contain the telephone number of a second custodial parent's place of employment; and the name and policy number of the child's medical insurance.
Plan of Correction: The provider shall request parents to provide the missing information.
