Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 03/22/2023. There were 3 children present with 2 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview, caregivers who work directly with children did not annually attend the department's health and safety update course.
Evidence:
1) There is no documentation in the record for Caregiver#2 of having completed the health and safety update course for the year 2022. The most recent training certificate in the record was dated 04/07/2018.
2) Caregiver #2 stated that they did not complete the training.
Plan of Correction: Moving forward, the provider shall ensure that Caregiver #2 completes all required training.
