Inspector Notes
A renewal inspection was conducted on 09/07/2023 between the hours of approximately 3:30 pm and 4:40 pm. There were 6 children present, ranging in ages from 3 years to 8 years, with 2 staff supervising. Children were observed while coloring, playing board games and free playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. No medication was present on the date of inspection. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 5Evidence:
1) On the date of inspection, the record for Caregiver #3 did not contain the results of a criminal history check or documentation that a criminal history check had been requested.
Evidence:
1) The record for Child #3 (start date 08/07/2023) did not contain the results of a physical examination.
Evidence:
1) The record for Caregiver #3 did not contain the following information: Date of employment, name address and telephone number of a person to be notified in an emergency, and documentation that two or more references as to character and reputation as well as competency were checked before employment.
Based on record review, the provider did not ensure that caregivers obtain and submit the results of a follow-up tuberculosis (Tb) screening at least every two years.
Evidence:
1) The Tb screening form in the record for Caregiver #2 was dated 02/10/2021.
Evidence:
1) The records for Child #1 and Child #2 did not contain the policy number of the child's medical insurance.
2) The records for Child #1 and Child #3 did not contain the work number for one custodial parent.
