Inspector Notes
A monitoring inspection was conducted on 04/29/2024 between the hours of approximately 10:00 am and 11:40 am. There were 3 children present, ranging in ages from 7 months to 3 years, with 2 caregivers supervising. Children were observed while playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
8Evidence:
1) On the date of inspection, the findings of the most recent inspection of the facility (09/2023) were not posted.
Based on record review, the Provider did not ensure that each child have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
1) The record for Child #1 did not contain the results of a physical examination.
Based on record review, caregivers' records did not contain all required information.
Evidence:
1) The record for Staff #3 did not contain documentation that a second reference as to character and reputation as well as competency was checked before employment.
Evidence:
1) The documentation shall have been completed within the last 30 calendar days of the date of employment. The Tb screening form in Staff #3's record (start date 02/26/2024) was dated 08/13/2020.
Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain: the child's first date of attendance; custodial parents' name, home address, phone number; work place name, address, and phone number; name, address and phone number of two people to call in case of an emergency; and information related to allergies.
2) The record for Child #2 and Child #3 did not contain the medical insurance policy number for the child.
Evidence:
On the date of inspection, two nonprescription medication, were observed in the fdh. Upon interview with the Provider, the following was determined: medication belonged to Child # 3 and Child #4; medication has been administered once, date was unknown; there was no written medication authorization form for either medication available for review.
1) Medication must be administered by a caregiver 18 years of age or older who meets the requirements in 8VAC20-800-220 A - The medication was administered by the Provider. The most recent MAT certificate available for review was dated with an expiration date of 12/13/2021.
2) Medication must be labeled with the child's name - Either medication was labeled with the child's name.
Evidence:
1) On the date of inspection, nonprescription medication for Child #3 and Child #4 were observed on a high shelf in the care area. The medication was not locked.
Evidence:
1) On the date of inspection, the Provider stated that the nonprescription medication observed in the fdh had been administered at least once. The Provider could not remember the date. A record of medication given to the children was not available for review.
