A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. The center is required to have employees undergo a background check every five years. The most recent central registry check for staff #1 and 2 was overdue by a year and ten months. The most recent central registry check for staff #5 was overdue by five days. The most recent central registry check for staff #6 was overdue by five months. The most recent fingerprint check for staff #6 was overdue by six months. The most recent sworn statement or affirmation for staff #1 was overdue by eight months. The most recent sworn statement or affirmation for staff #2 was dated 2/9/03 and therefore past due for update. The most recent sworn statement or affirmation for staff #5 was overdue by two months.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #4, who had been employed for six months, did not have a completed national criminal background check.

Each child?s record shall contain the name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached. The record for child #1 who has been enrolled for two months did not contain any emergency contact information.

Standard 8VAC20-790-760-B-3-c
A nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing. Staff stated that children wearing pull-ups are changed while standing in front of the toilet and not a nonabsorbent surface for diapering and changing.
