Inspector Notes

An unannounced, on-site renewal inspection was initiated on 03/20/2025 and completed on 03/24/2025 as a part of the provisional licensure period. The on-site inspection began at 9:50 AM and ended at 2:30 PM. The inspector reviewed compliance in the areas listed above. There were 146 children present and 23 staff. The inspector reviewed 10 children?s records, 10 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

There was a pacifier in the infant room that was not labeled with the owner?s name.
Plan of Correction: The administration will work on a procedure to ensure that all children's personal articles are labeled with the owner's name.
