The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. There was no documentation of an out of state sex offender check in the record of Staff #4 (employed for 10 months).
Plan of Correction: We will use a different database that was provided during the visit instead of the one previously used which showed no results. The administrator will recheck new hires to ensure documents and background checks are completed within the guidelines

The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Immunization documentation for Child #2 and Child #3 show that the documentation was not received every six months, required for children under the age of two.
Plan of Correction: Files will be reviewed for new immunizations semi-annually. The parent was notified immediately for an update.

Plan of Correction: Employee was sent during her break to do her screening . Employee was on medical leave when her TB test expired. The Assistant Director will recheck files semi-annually to ensure all files are updated and in compliance.
