During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/4/2026 and completed on 3/4/2026. The on-site inspection began at 9:56 am and ended at 11:17 am. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 21. The inspector reviewed five children?s records and two caregiver on-site on 3/4/2026. This inspection included document review , tour of the facility, interviews and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/17/2026. A POC submitted after this date will not appear on the public website.

The program is required to submit to VDH and implement a plan to test potable water for lead.

The family day home shall obtain documentation of additional immunizations for a child once every six months for children under the age of two years.

Child #3, 12 months old, last documented immunization was over six months ago and 'Child #5, 10 months old, last documented immunization was over six months ago. Child #3 and Child #5 were in care during the inspection and had been in care for seven months.
