Inspector Notes

An unannounced monitoring inspection was conducted on May 23, 2024, from approximately 11:50am until 1:00pm. There were 11 children present, ranging in ages from 3 months to 3 years, (30 points) with 2 caregivers supervising. The inspector observed children handwashing and eating lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 3 caregiver records and 1 household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on record review and interview, the provider did not ensure that all staff obtained a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1. The record for caregiver #1, caregiver #2, and caregiver #3 did not contain documentation of a minimum of sixteen clock hours of training annually in areas relevant to their responsibilities.
2. Caregiver #1 confirmed that sixteen hours of training had not been completed for caregiver #1, caregiver #2, and caregiver #3.
