Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 04/23/2021 and concluded on 04/29/2021. The provider was contacted by telephone to initiate the inspection. There were 11 children present and 2 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard 22VAC40-111-100-A
Based on record review and interview with the provider, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1) The record for Child #1 (start date 01/29/2018) did not contain documentation of a physical examination.
2) The provider could not locate the document in Child #1's record.
Plan of Correction: The provider shall request parents to provide a physical examination for Child #1.
