During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated on 8/25/2025 and completed on 8/25/2025, as a part of the licensure period. The on-site inspection began at 8:45am and ended at 10:01am. The inspector reviewed compliance in the areas listed above. There were nine children present and three caregivers; the point total was twenty one. The inspector reviewed five children?s records and five caregiver/household members records on-site. This inspection included document review and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3One of two reference checks for Caregiver #3 was checked thirteen days after employment.
A tuberculosis (TB) screening is required every two years from the last screening.
An updated TB screening for Caregiver #2 was not completed and was overdue by 150 days.
Records for children #2, #3, #4 and #5 did not have a parent?s signature for an annual review of the emergency contact information. Children #2 and #5's were outdated by four months. Child #3's was outdated by six months and Child #4's by eight days.
