During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 02/10/2026 and completed on 02/10/2026, as a part of the licensure period. The on-site inspection began at 9:15am and ended at 10:38am. The inspector reviewed compliance in the areas listed above. There were twelve children present and three caregivers; the point total was thirty-three. The inspector reviewed five children?s records and five caregiver/household members records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Caregiver #2 was not completed and was overdue by ten months.
Plan of Correction: Will obtain documentation of TB screening for caregiver #2.
Standard 8VAC20-800-680-B
Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after toileting or diapering.

Child #5's hands were not washed with soap and water after diapering.
Plan of Correction: Will review toileting procedures and make sure children's hands are washed with liquid soap and warm running water after diapering.
Standard 8VAC20-800-690-D
The diapering surface shall be nonabsorbent and washable.

Both diaper pads in the family day home had torn surfaces.
Plan of Correction: Will purchase new diaper pads.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.

The monthly emergency evacuation procedure was overdue by one month.
Plan of Correction: Will practice the evacuation drill every month.
