Inspector Notes

The licensing inspector conducted an unannounced complaint investigation in response to a complaint that was received by OCCHS on May 10, 2023 regarding supervision, ratio and physical plant. The inspection was initiated on May 15, 2023 and completed on May 30, 2023. On May 15, 2023, the inspector was on site from approximately 11:45pm to 3:37pm. Interviews with staff were completed and documentation was reviewed. Additional interviews were completed on May 30, 2023.
The preponderance of the evidence gathered during the investigation supports a part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Based on observation, the center did not ensure to implement their established procedures regarding supervision.
Evidence:
1. The center representative stated that children should always be within sight and sound of staff.
2. The inspector observed three infant and toddlers, sleeping in cribs in a dark room with the door closed. The infants were alone and unsupervised for approximately 45 minutes. After the inspector notified the teacher and center representative, the inspector continued to observe a staff member leave in and out of the infant room where children were asleep, leaving them out of sight and sound supervision. No other staff member was observed in the crib room.
Plan of Correction: I sat the teacher and the others in the classroom down and went over expectations and guidelines again. I asked if there were any questions pertaining to what was said they stated they understood. The teacher was written up and placed on a 30-day probation. [The center director] and/or [the assistant director] will monitor cameras and do rounds to ensure safe sleep practices.
