Inspector Notes

An announced initial subsidy health and safety inspection was conducted on November 21, 2023 from approximately 10:00 am to 12:15 pm. There were 57 children in attendance and a total of 11 staff present. 9 staff records and 5 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed getting ready for lunch, coloring, dancing, getting diapers changed, and playing with blocks while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 11/22/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on record review, the vendor failed to ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.

Evidence:
1. The record for staff #1, date of employment 05/14/2018, did not contain documentation of TB screening.
2. The record for staff #3, date of employment 08/08/2022, did not contain documentation of TB screening.
3. The record for staff #5, date of employment 01/14/2019, did not contain documentation of TB screening.

Based on observation, the vendor failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. There was an aerosol can of Lysol Disinfectant spray on top of the paper towel holder in the restroom.
2. There was a can of air freshener spray on top of a shelf in classroom #1.
3. There was a large bottle of hand sanitizer on top of a desk in classroom #2.
