During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 9/15/2025 and completed on 10/6/2025, with local agencies, in response to a complaint received by the licensing office on 9/9/25 relating to supervision. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/17/2025. A POC submitted after this date will not appear on the public website.

1. On August 29, 2025, Staff #1 left a group of 16 children (ages 3-school age) unsupervised in a classroom for at least 2-3 minutes.
2. During the inspection on September 15, 2025, the preschool room (3-4year olds) was observed with one staff supervising 19 children for approximately 5 minutes.
3. We have communicated to our staff that if a teacher leaving the room to answer the door creates a ratio issue, then check to see who it is from the preschool door if it?s something that can wait, then ask the person to return later. If it is important then reach out to administration from the tablet and we will handle things accordingly. All children should and shall remain in sight and sound supervision.
On August 29, 2025, Staff #1 knowingly left child #1 (age 8) outside of the building in a play area unsupervised for at least 2-3 minutes. Child #2 (age 3) was also left outside of the building in a play area unsupervised for at least 2-3 minutes, however, staff #1 was unaware this child was unaccounted for.
