Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/17/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-570-3
Any suspected incident of child abuse or neglect shall be reported in accordance with ? 63.2-1509 of the Code of Virginia.

Staff #2 and Staff #3 acknowledged the incident on August 29, 2025 with Staff #1 was a suspected incident of child neglect and failed to report the incident to Child Protective Services.
Plan of Correction: 2. Center will report all necessary incidents, and/or accidents to the appropriate entities within the correct time period, same day after the report is given to the parent, or guardian.

Children less than 10 years of age shall always be within actual sight and sound supervision of staff.

On August 29, 2025, Staff #1 left child #1(age 3) and child #2 (age 8) outside of the building in a play area unsupervised for at least 2-3 minutes. On August 29, 2025, Staff #1 left a group of 16 children (ages 3-school age) unsupervised in a classroom for at least 2-3 minutes.
Plan of Correction: 1. Staff #1 was terminated due to not following protocol, and endangering children. Staff #1 had a walkie talkie where she could have reached out to administration if she needed assistance and chose not to. We have since emphasized the importance of communication and using the proper resources to ensure we are communicating effectively.
