During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 9/23/2025 and completed on 10/6/2025, with local agencies, in response to a self-report received by the licensing office on 9/16/2025 relating to supervision. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the report is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/17/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.031-A-3-a
The child day center shall employ supervisory personnel according to the following ratio of staff to children: One staff member to four children from ages zero to 16 months. When a group of children receiving care includes children from different age brackets, the age of the youngest child in the group shall be used to determine the staff-to-children ratio
that applies to that group.

During the inspection on September 23, 2025, the infant room was observed with 2 staff supervising 10 infants for approximately 5 minutes. Staff #3 confirmed this room was out of ratio for approximately 1 hour due to an additional staff being on break.
Plan of Correction: 3. We are currently hiring more staff to accommodate the shortage during breaks.

When a group of children receiving care includes children from different age brackets, the age of the youngest child in the group shall be used to determine the staff-to-children ratio
that applies to that group.

During the inspection on September 23, 2025, the toddler room (16 months to 2 years) was observed with one staff supervising 8 children for approximately 5 minutes. Staff #3 confirmed this room was out of ratio for approximately one hour due to an additional staff being on break.
Plan of Correction: 2. We are currently hiring more staff to accommodate the shortage during breaks.

Repeat Violation
The center shall establish and implement procedures for: Appropriate supervision of all children in care.

On September 15, 2025, video footage shows that Staff #1 left 12 children (ages 2 years to 5 years) unsupervised in a room for approximately 8 minutes. During this time, child #2 (age 4) was attacked by multiple children resulting in an injury that required medical attention.
Plan of Correction: 1. Staff #1 was suspended due to administration not being notified immediately and upon return is placed on a probationary period as training continues, due to the process of hiring more staff. It has been made extremely clear no child or children are to be left alone or not within sight and sound supervision. This conversation was also had with all other staff members as well.
