Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/17/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
Children less than 10 years of age shall always be within actual sight and sound supervision of staff.
On September 15, 2025, video footage shows that Staff #1 left 12 children (ages 2 years to 5 years) unsupervised in a room for approximately 8 minutes. During this time, child #2 was attacked by multiple children resulting in an injury that required medical attention.
Plan of Correction: 2. Staff #1 was suspended due to administration not being notified immediately and upon return is placed on a probationary period as training continues, due to the process of hiring more staff. It has been made extremely clear no child or children are to be left alone or not within sight and sound supervision. This conversation was also had with all other staff members as well.

The vendor shall notify the parent immediately when a child sustains a serious injury.
On September 15, 2025, child #2 sustained a serious injury to his eye and the parent was not notified by the center staff.
Plan of Correction: 1. All staff has been informed that if a child sustains an injury at the center administration and the parents are to be notified immediately. If administration isn?t in the building due to drop offs in the morning or afternoon, then a picture needs to be sent with a phone call to include an explanation of the situation. So that administration can reach out to the parent. If footage can be viewed it will be, and a proper explanation will be provided to the parent; and all other entities that would need to be notified per situation.
