The child day center shall maintain supervisory personnel according to the following ratio of staff to children: The required ratio for children ages zero to 16 months is one staff member to four children. When a group of children receiving care includes children from different age brackets, the age of the youngest child in the group shall be used to determine the staff to children ratio that applies to that group.

In the Infant room, there were eight infants, ages four months to 13 months. One staff was observed taking one infant across the hall into another room for a diaper change, leaving one staff with seven infants. Administration stated this is the normal routine to change diapers in the other classroom.

In classroom #1 (16 months through two years), two staff were supervising 12 children. Child #1, 15 months in age, was present in the classroom. One additional member of staff was required.
Plan of Correction: Per administration: Will make sure communication is made when late from break and bathroom breaks.

In classroom #2 (two years-three years), one staff member was supervising 12 children. One additional staff was required.

The center did not ensure compliance with the terms of the religious exemption issued by the Virginia Department of Education. The center?s current exemption permits services to 74 children in the age range from 0 years, six months through 12 years, 11 months. Administration confirmed that child #4, four months in age, was enrolled in the program.
Plan of Correction: Per administration: We will request to change the age range.

Repeat
The center shall establish and implement procedures for: Appropriate supervision of all children in care. Multiple children, ranging in age from three to four years, were observed walking unattended to the bathroom. The bathroom was located through the kitchen and in the corner which did not have sight and sound supervision from the classroom. Children walked by accessible cleaning products sitting on the counter and the stove that had six burners with open flames.
Plan of Correction: Per administration: Will make adjustments to bathroom supervision due to maintenance.
