The child day center shall maintain supervisory personnel according to the following ratio of staff to children: The required ratio for children ages zero to 16 months is one staff member to four children.
Upon arrival, there was one staff supervising five infants, ages five months to 12 months. A second staff entered the room approximately 1-2 minutes later to meet ratio requirements.

Plan of Correction: We hosted a training on a closed day to go over protocol and gave each classroom a ratio sheet to put up in their room. All staff have been structed if they are at ratio and must leave the room, they are to find relief by calling their walkie talkie prior to leaving the room. If there is only one teacher in the room and they receive another child that would put them out of ratio they are to call on the walkie to let of the floaters know to come into their room to assist to always ensure ratio moving forward. They are aware if they are found out of ratio and no one has reached out then there will be a suspension to follow or termination depending on the circumstance.

The center?s current exemption permits services to 74 children in the age range from 0 years, six months through 12 years, 11 months. Administration confirmed that child #2, five months in age, was enrolled in the program.

Plan of Correction: We thought that we put 6wks to 12 years on our paperwork. We will submit this year's renewal as 6weks to 12years 11 months to better serve our community moving forward.

The vendor shall maintain and keep at the center a record for each enrolled child, which shall be made accessible to the department's representative upon request.
The record for child #2 was unable to be provided to the licensing inspector during the inspection.

Plan of Correction: Child #2 file was on the desk and not in the bucket of files we have for our new coming children. The director call the assistant director and informed where the file was. Inspector was already gone; Director called to inform the inspector the file was here on the desk. We will ensure files are in the proper place moving forward.
