During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on 3/26/2026 and completed on 3/30/2026 in response to a self-report received by the licensing office on 3/26/2026 relating to supervision. The Department issued a Notice of Intent to revoke the exemption which had been granted to Cornerstone Ministries of Virginia, Inc. to operate Cornerstone Tender Care, LLC on November 21, 2025, as a result of high-risk violations. The inspector reviewed 1 staff record on-site. This inspection included tour of the facility, document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable code sections, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/9/2026. A POC submitted after this date will not appear on the public website.
Violations
1**Systemic Deficiency**
The center shall establish and implement procedures for: Appropriate supervision of all children in care.
The center's policy for drug and alcohol use states, "No employees or individual who performs work for Cornerstone Tender Care LLC is allowed to consume, possess, sell, purchase or be impaired by alcohol or illegal drugs, as defined under federal and/or state law, on any property owned by or leased on behalf of Cornerstone Tender Care LLC."
On March 24, 2026, staff #1 was observed by multiple staff members to have abnormal behavior between 8:12am to approximately 2:18pm, including stumbling or unable to stand up, glassy eyes, disorganized speech, and frequently requesting to leave their room. Staff #1 was found to have alcohol on the premises, and multiple staff suspected staff #1 to be inebriated. Management believed they detected the odor of alcohol on staff #1 at approximately 9:30am and had been previously informed that staff #1 had a history of alcohol use in the workplace while employed by a previous employer.
Additional information revealed during the investigation related to staff #1?s interactions and supervision of children on March 24, 2026 includes:
1. Staff #1 threw a shoe at a child (under age 2) after witnessing the child playing with staff #1?s phone that had been left on a nearby table.
2. Staff #1 was observed by two staff members to have an interaction with a child (3 years old) where staff #1 attempted to stop the child from walking in the lunchroom. Staff #1 reportedly placed her hand in front of the child?s chest to keep the child from moving forward and caused the child to fall and spill juice on themselves.
3. Staff #1 was alone supervising 9 toddlers (ages 16 months to 2 years) during rest time from 1pm to 2pm.
