Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/9/2026. A POC submitted after this date will not appear on the public website.
Violations
2On March 24, 2026, Staff #1 was observed by two staff members to have an interaction with a child (3 years old) where staff #1 attempted to stop the child from walking in the lunchroom. Staff #1 reportedly placed her hand in front of the child?s chest to keep her from moving forward and caused the child to fall and spill juice on themselves.
On March 24, 2026, staff #1 threw a shoe at a child (under age 2) after witnessing the child playing on staff #1?s phone that was left on a nearby table.
On March 24, 2026, Staff #1 was observed by two staff members to have an interaction with a child (3 years old) where staff #1 attempted to stop the child from walking in the lunchroom.
