During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on May 9, 2025. The on-site inspection began at 10:07am and ended at 11:25am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 27. The inspector reviewed 5 children?s records and 2 caregiver records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on May 21, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-140-B-1
Providers licensed after and substitute providers employed after June 30, 2010, shall have a high school program completion or the equivalent or (ii) evidence of having met the requirements for admission to an accredited college or university.

The record for Caregiver #1, employed for years 9 years, who was working as a substitute provider at the time of inspection, did not have evidence of a high school program completion or the equivalent or evidence of having met the requirements for admission to an accredited college or university.
Plan of Correction: Caregiver #1 has completed an international high school program abroad. While this institution is no longer operating, the facility is reaching out for available documentation to demonstrate completion of a high school program. If unsuccessful, the Provider will elect an alternate substitute provider that meets VDOE Division of Licensing Programs requirements in the event of any future need and use of a substitute provider. Historically, Caregiver #1 has not been delegated as the substitute while the primary Provider has been away. This would be the first occurrence of using Caregiver #1 as a substitue provider, thus failure to verify all required documentation was available on file is an oversight on behalf of the facility.

Children's records are required to contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

The record for Child #1 was missing the address for two designated persons. The child was present during the inspection and had been in care for nine months.
Plan of Correction: An additional layer of review will be implemented at submission of new child registration documents, to include a sign-off sheet that all VDOE Division of Licensing Programs mandated information has been provided by the family and collected by the Provider.
