Based on a staff written statement and review of an injury report, the licensee did not ensure compliance with the center's own policy on playground safety.
Evidence:
1. The center's playground safety policy requires staff to spread out in zones where there is playground equipment.
2. On 04/25/2024 at approximately 4:25pm, Staff #1 and #2 were positioned in two different zones, when Staff #1 left the playground and entered the center, which left one zone without staff.
3. While Staff #1 was inside, Child #1 fell off a climbing structure which caused a serious injury. The climbing structure was located in the zone that Staff #1 had left from.
