Inspector Notes

An unannounced monitoring inspection was conducted on 12/04/23 from 7:25 a.m. to 9:55 a.m. with the Director. There were 39 children in care, ranging in age from eight months to five-years-old, supervised by 20 staff. The children were observed in free play. Five child records and five staff records were reviewed. Four medications and authorization forms were reviewed. Required postings were observed. The attendance, and emergency drill log were reviewed. The first aid kit, flashlight, and battery- operated radio were observed. The most recent Fire Inspection on file was dated 11/27/23 and the most recent Health Inspection on file was dated 03/08/23. Areas of non-compliance are identified in the Violation Notice. Repeat Violation

Based on observation, the center did not ensure that where playground equipment was provided, resilient surfacing was maintained in fall zones.

Evidence: There was less than one inch of mulch around the composite play structure on the preschool playground.
Plan of Correction: Mulch has been raked to the correct depth in fall zones. We are researching alternate resilient surfacing options.

Based on review of the written attendance record, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurs.

Evidence:
1. In the Bumblebee?s classroom, ages 15 months to 2-years-old, there were eight children present in the classroom, the written record reflected there were six.
2. In the Panda?s classroom, ages three-years-old, there were seven children present in the classroom, there was no written record of attendance.
Plan of Correction: Attendance records will be maintained upon arrival and departure in all classrooms each day.
