During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 01/03/2025 and completed on 01/17/2025 with a local agency in response to a self-report received by the licensing office on 12/27/24 relating to behavior guidance, forbidden actions, and equipment and materials. The inspector reviewed four children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the investigation determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/28/25. A POC submitted after this date will not appear on the public website.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior.

For two months, staff in the Dolphin?s classroom placed Child #2 in a ?buckle chair?, a chair with a belt that fastened around the child?s lap, at circle time to prevent them from running and climbing during instruction time.
Plan of Correction: Chair was used with a teacher so student remained safe and included in activities of circle time. Student was able to easily buckle and unbuckle themself. Chair was removed from room.

The following actions or threats thereof are forbidden: Enclosure in a small confined space or any space that the child cannot freely exit himself.

In the Dolphin?s classroom, two-years-old, staff used a ?buckle chair" for Child #1 at lunch time to prevent them taking food off of other children?s plates.
Plan of Correction: Chair was used twice for a child's safety to prevent them from ingesting any allergens that would cause severe reactivity.
