During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced investigation was initiated on 3/10/25 and completed on 3/25/25 in response to a complaint received by the licensing office on 2/28/25 relating to allegations of care, protection, and guidance, and forbidden actions. The inspector reviewed one child record and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation of forbidden actions; therefore, the complaint is determined to be valid. Though not part of the original complaint, violations that were not reported were found related to administration. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/15/25. A POC submitted after this date will not appear on the public website.
Violations
2Evidence: The center?s policy on cell phone use states, ?Employees may us their cellular phones in a non-disruptive manner during rest time, when all students are asleep.? In April of 2024 a staff member, in the toddler class, was observed using a multimedia messaging app during rest time. The staff member was sitting on the floor with their legs across the back of a child, actively using their cell phone while children were off their cots and walking around the classroom.
The following actions or threats thereof are forbidden enclosure in any space that the child cannot freely exit himself.
Evidence: In April of 2024 three different staff members working in the toddler classroom, ages 16-24 months old, would put their legs across the back of an eighteen-month-old child during rest time to prevent them from getting up from their cot. It was reported that the teachers did this for two months.
