Inspector Notes

An unannounced monitoring inspection was conducted on May 24, 2024 approximately 12:30 pm to 1:30 pm. There were 9 children for a total of 7 points directly supervised by 1 staff. The home, 1 staff records, 2 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely. There was an assortment of age appropriate materials for children.

Based on children records review, 2 out 2 records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:

Based on household and staff records review, 2 out 2 records, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
