Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 23, 2023 from approximately 10:00 AM to 12:30 PM. There were 15 children in attendance and a total of five staff supervising. A total of five children and four staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-560-A
Based on the review of five children's records, the vendor did not obtain documentation that two of the children had received the immunizations required by the State Board of Health before allowing the children to attend the center. Evidence:
1. The records for Child #3 (first day of attendance 08/15/2023) did not have documentation of immunizations.
2. Staff #4 confirmed this information during the inspection.

Based on observation and interview, the vendor did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
The cabinet in the children?s bathroom contained cleaning chemicals. The cabinet did not contain a safe locking method.

Based on observation, the vendor did not ensure children?s hand were washed with soap and running water after toileting.
Evidence:
A staff member in the Infants? classroom was observed changing a child?s diaper. After the diaper change, the staff member did not wash the child?s hand. The child was placed back on the floor to play.
