During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site code compliance inspection was initiated on 10/16/24 and completed on 10/21/24. The on-site inspection began at 11:00am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 18 children present with seven staff. The inspector reviewed five children?s records and five staff records on-site. Additional documentation was reviewed electronically on 10/21/24. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance(s) with applicable code sections, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/14/24. A POC submitted after this date will not appear on the public website.
Violations
4Staff #3 and Staff #4 were in a supervisory position for two and three months prior to having a completed staff health report.
The staff health report in the record of Staff #5, who is in supervisory position, was not completed annually. The staff health report was expired by more than one month.
The most recent national fingerprint criminal background check for Staff #1 was overdue by a year and six months and their most recent central registry check was overdue by a year and two months.
The national fingerprint criminal background check for Staff #2 was completed more than seven months after their first day of employment.
Staff #5, who had been employed for over eight months, did not have a completed national fingerprint criminal background check.
The results of the central registry search in the record of Staff #2 were obtained nine months after employment.
Staff #3 did not have central registry search results during the inspection and has been employed for over seven months.
The results of the central registry search in the record of Staff #4 and Staff #5 were obtained more than six months after employment.
