During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 4/15/2025. The on-site inspection began at 8:34am and ended at 9:25am. The inspector reviewed compliance in the areas listed above. There were three children present and one caregiver; the point total was six. The inspector reviewed two children?s records and one caregiver record on-site on 4/15/2025..
This inspection included document review, tour of the home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/22/2025. A POC submitted after this date will not appear on the public website.
Violations
7The Assistant did not meet the requirements for a substitute provider and was caring for children for more than two hours when the provider was absent from the home. The Assistant stated the provider would be returning tomorrow.
One unused outlet under the Alphabet Center area was not covered with a protective cover.
The supplemental page from the 5/30/2024 inspection and 12/03/2024 inspection were posted on the parent bulletin board. This page is considered confidential.
The record for Child #1 and Child #2 were missing the employment work phone numbers for both parents and the insurance policy number..The children were present during the inspection and had been in care for one year and 7 months,.
The provider's first aid kit was missing the 2nd triangular bandage.
The assistant stated the responsible person for emergency backup care was not posted.
The Assistant stated she did not know where the emergency preparedness and response plan was located.
