Inspector Notes
A monitoring inspection was conducted on 7/25/2023. The inspector arrived at 9:50 AM and departed at 11:10 AM. There were 99 school aged children present with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

1. The record for staff 2, employed since 6/23/2023, did not include the results of a central registry search.
2. Staff 1 acknowledged that the record for staff 2 did not include the results of a central registry search.
The results of a central registry search were requested in a timely fashion, however, we will follow up to determine when the results can be expected or if another request must be submitted.
Evidence:
1. The record provided for child 1, who began attending on June 25, 2023, did not include the results of a physical examination.
2. Staff 1 acknowledged that child 1's record did not contain the results of a physical examination.
The parent was immediately contacted to scan a copy of the child's physical examination to us. The physical exam documentation was received the same day as the inspection.
