Inspector Notes

A renewal inspection was initiated and concluded on 07/03/2024. The inspector arrived at 12:40 PM and departed at 2:23 PM. There were 73 children present, ranging in ages from 6 yrs to 12 yrs, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using a safe locking method that prevents access by children.
Evidence:
There was a container of sanitizing wipes with a warning label on an open shelf behind a cloth curtain in the 2nd grade classroom.
Plan of Correction: The provider responded with the following:
Staff will be reminded to keep all chemical based materials locked up at all times and to make a check of each space at the beginning and end of each day - and throughout the day after these have been used.

Based upon review of children's injury records, the facility has not ensured that the written record of children's injuries includes all required information.
Evidence:
An injury record dated 7/3/2024 for child 6 did not include the time of the child's injury nor the date the parent was notified of the injury.
Plan of Correction: The provider responded with the following:
Site Director will ensure staff is aware of the need to complete each item on the report - including date/time and date when parent was called\Ex. Director and Ed. Director will double check each report to ensure accuracy

Based upon review of children's records, the facility has not ensured that the records include the business telephone numbers for parents/guardians.
Evidence:
The records for child 1, 2, 3, 4 and 5 did not include business telephone numbers for the parents as the program used to enroll children does not ask for that information.
