Inspector Notes

An unannounced monitoring inspection was conducted on 06/10/2024. There were 4 children, ages 6 years- 7 years, totaling 4 points, and 1 caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, 1 caregiver record, and 1 household member records were reviewed. The children were participating in free choice activities and in arts and crafts. The inspector arrived for the inspection at 9:30 am and departed at 10:25 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the provider failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence: The most recent immunization record available for Child 5 (20 months old) was dated 05/25/2023. Documentation of additional immunizations were not obtained every six months for a child under the age of 2 years.
Plan of Correction: Will notify the parents today.
