Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 05/20/2025. The on-site inspection began at 3:00 pm and ended at 3:50 pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 6. The inspector reviewed 4 children?s records and 2 caregiver/household member records on-site.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

To assist in preventing choking, food that is hard, round, small, thick and sticky, or smooth and slippery, such as uncut grapes and popcorn, shall not be served to children under four years of age.

On 05/20/2025, Child 1 (20 months), ate uncut grapes and popcorn.
Plan of Correction: Will no longer serve to the child.
