Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 9/13/23. At the time of inspectors' arrival there were 14 aged children in care with 5 staff members and 1 administrator. Children were observed interacting with staff, educational activities, and lunch. A sample of 5 children's records and 4 staff records were reviewed. Injury reports were also reviewed. Director reported that medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review it was determined the center did not ensure that all staff complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Evidence:
1. The records for staff #2(hired 6/20/23) and staff #5(hired 6/20/22) did not contain documentation of orientation within the required time frame.
Plan of Correction: Staff will ensure that orientation is done within the required timeframe.
Standard 8VAC20-780-550-P
Based on record review and interview it was determined the center did not ensure that the written record of children's serious and minor injuries contained all of the required components.
Evidence:
1. The injury reports for child #6 and child #3 did not contain the circumstance of the injury and future action to prevent recurrence.
2. Staff confirmed that this information was not on the injury log available during this inspection.
Plan of Correction: Staff will be reminded to include all required components on the injury log.

Based on record review and interview it was determined the center did not ensure that the
record for each child contained all of the required information.
Evidence:
1. 5 out of 5 records did not contain the names of any previous or concurrent day care and/or
schools attended by the child.
2. Child #4's record did not contain documentation of their mother's work information.
3. Staff confirmed that this information was not available during this inspection.
