Inspector Notes

An unannounced monitoring inspection was conducted on July 5, 2023. There were a total of 25 children with 4 staff members supervising. Children were observed engaged in various age appropriate activities, outdoor activities, bathroom procedures, and eating lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of 5 child records and 6 staff records were reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-350-O
Based on observation, the staff failed to maintain a ratio of one (1) staff member for every 14 children in a balanced-mixed-age grouping of children.
Evidence:
1. At the time of inspection (12:20pm) the Holly Classroom had 17 children with one staff supervising (Staff #2).
2. Staff #5 walked into the Holly Classroom at 12:27pm
3. Staff #5 acknowledged leaving the classroom to go to the office.
Plan of Correction: We had 3 staff today. I will speak to the staff about maintaining ratio.

Based on medication review, expired medication was returned to the parent for disposal.
Evidence:
1. Child #4 medication (Epi Pen) expired 05/2023.
2. Child #5 medication (Epi Pen) expired 06/2023.
Plan of Correction: I will ensure expired medication is sent home.
