During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on July 2, 2025. The on-site inspection began at 11:05am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 36 children present and seven staff. The inspector reviewed four children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-A
Repeat Violation: The center is required to have employees undergo a background check every five years. The most recent central registry check for staff #2was overdue by one month and the national fingerprint was overdue by two months.
Plan of Correction: The background checks will be completed by 7/9/25.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #1 updated screening was due two months ago. The updated screenings have not yet been completed.
Plan of Correction: Staff will have the TB screening completed by 7/7/25.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The metal fence on the right side of playground was broken due to a tree falling on the fence. There were sharp edges exposed where the rails had broken.
Plan of Correction: We will secure the area until contractor repairs the fence. Children are not allowed to play in the area.
Standard 8VAC20-780-290-A-3
In areas used by preschool age or younger children, electrical outlets shall have protective covers. Children in classroom #1, ages 3-6 years, were laying on their mats near one uncovered outlet that was within their reach.
Plan of Correction: Staff installed a protective cover. Violation was corrected during inspection.
