Inspector Notes

This inspection was conducted by licensing staff using and alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on October 15, 2021 and concluded on October 25, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 34 children present directly supervised by 5 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The children were observed in circle time. The inspector reviewed on two children records and one staff record.

New requirements became effective on October 13, 2021. The facility has not yet fully complied with the following requirement(s):
-develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.

The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff records, the child day center did not obtain a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #2 (date of employment 8/25/2020) did not have a out of state child abuse and neglect request by the end of the 30th day of employment.
2. Staff #2 did not have a out of state criminal history name check required for the state has resided in the past 5 years prior to the first day of employment.
Plan of Correction: We have contacted state of Wisconsin, summited forms. We are awaiting their response.
