Inspector Notes

An unannounced, on-site renewal inspection was initiated on 1/15/2025 and completed on 1/15/2025. The on-site inspection began at 11:25am and ended at 12:50pm. The inspector reviewed compliance in the areas listed above. There were 41 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Prescription medication may be administered if the parent has completed a written authorization from a parent or guardian. There was no written parent/guardian or physician authorization to administer prescription medications to child #5.
Plan of Correction: Center will obtain written authorization from the parent to administer the medication.
