Inspector Notes
An unannounced monitoring inspection was conducted today between 1pm and 2pm. There were 11 children present with the Provider and 1 assistant for a total of 20 points within the supervision guidelines. The toddlers were observed resting and the school aged children were observed watching tablets and playing with manipulative toys. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
6Evidence: Household member #1's most recent TB screening is dated 3/7/2019.
Evidence: The Supplemental page from the most recent inspection was observed to be posted containing children's and staff confidential information. The top of the Supplemental page clearly stated "DO NOT POST" and was signed by the provider.
Evidence: The provider did not have a record for Child #3. The provider stated that the child was dropped off unexpectedly and they did not request a record because they have a record for Child #3's sibling.
Evidence: The provider stated that the younger children went to nap at approximately 1pm. At approximately 1:45pm a small training toilet that had not been emptied and cleaned was observed in the children's bathroom.
Evidence: The provider stated that Child #1 and Child #2 bring lunch boxes to the family day home with food from home. Both lunch boxes were not labeled with the children's names or today's date.
Evidence: The provider stated that they cook and provide food for most of the children in care. There is no menu posted for the children. The provider stated that they have an online menu created each week. The provider confirmed that the parents do not have access to view the menu.
