Inspector Notes
An unannounced renewal inspection was conducted today between 11:05am and 12:30pm. There were 11 children present with the Provider and 1 assistant for a total of 29 points within the supervision guidelines. The children were observed playing freely, watching a Disney movie, washing hands and eating lunch (macaroni and cheese, green beans, grapes and milk). A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. The provider is not trained to administer medication and confirmed that there were no medications kept on site. Violations were found during today?s inspection and can be viewed on the Violation Notice.

5Evidence: Household member #2's most recent TB screening is dated 12/3/2020.
Evidence: Caregiver #1 and Caregiver #2 have documentation of 12.5 hours of training completed in 2022. Caregivers confirmed that they did not obtain 16 hours.
Evidence: A tube of diaper ointment was observed on the diaper changing station without a child's name on it. Caregivers confirmed the diaper ointment belonged to Child #1.
Evidence:
1. Child #1 (14 months) most recent immunizations are dated from when the child was 2 months old.
2. Child #5 (15 months) most recent immunizations are dated from when the child was 4 months old.
Evidence: The provider stated that they provide food for the children in care. There is no menu posted.
