Inspector Notes

An unannounced code compliance inspection was initiated on 7/18/2022 and concluded on 7/18/2022 from 10:20 AM to 11:20 AM. There were 11 children present, ranging in ages from four to 10, with two staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, procedures and medication. A total of two children?s records and two staff?s records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to obtain fingerprint-based criminal history check determination letters prior to the first day of employment.

Evidence:

1. Per the administrator staff 1 started work on 6/14/22 but was not left alone with children for several days.
2. The results for the fingerprint background check are dated 6/16/22.
Plan of Correction: In the future no staff will be hired until a fingerprint background check determination letter is received.
