Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 19, 2023 from approximately 9:20 AM to 11:45 AM. There were 21 children in attendance and a total of three staff supervising. A total of five children and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-550-2-f
Based on review of five staff records and interview, the vendor did not ensure one staff member obtained documentation of subsequent background checks at least every five years.
Evidence:
1. The record for Staff #5 (Date of Employment 07/27/2015) contained satisfactory results of the fingerprint-based national criminal background check dated 04/14/2018, a sworn statement dated 03/28/2018, and the central registry finding could not be located during the inspection.
2. Staff #3 confirmed the information during the inspection.
Plan of Correction: We will obtain the required updated background checks.
Standard 8VAC20-790-640-A
Based on observation, the vendor did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevented access by children.
Evidence:
Cleaning chemicals were in the cabinet underneath the sink in the bathroom. The cabinet did not contain a locking method. Children were observed using the bathroom during the inspection.
